Scheduling

To schedule an assessment, we will need the following forms completed and returned to_1935 Medical District Drive EL.02, Dallas, TX 75235_. Forms can be faxed to 214-867-5461. Once the forms are returned, you will be contacted by our office to schedule your appointment. 

Pediatric History Questionnaire​ - This document asks questions about your child. Please complete this questionnaire and return it as soon as possible. (English |  Spanish)

Record Release – This document gives us permission to request school records (ARD information, Full Individual Evaluation) from your child’s school. Please complete and return as soon as possible to 1935 Medical District Drive EL.02, Dallas, TX 75235.
