Transitioning to adult care

Technology offers tools to help you manage your transition journey. Children’s Health℠ encourages the use of MyChart, an online medical records system used by many major health care systems.

Through the MyChart patient portal, you can access your medical record and history through the transition from Children’s Health to adult care.

• Starting at age 14, a patient can sign up for their own MyChart account.

• At age 18, a parent’s access to their teenager’s MyChart account will end.

  • If continued access to a MyChart account is needed, contact the HIM department and provide a copy of legal medical decision-making documents.

• At age 18, a patient can share their Children’s Health MyChart record by choosing to give access to friends and family.

My Asthma Buddy makes it easy for children and families to track the necessary information to help monitor and manage their asthma symptoms effectively over time, playing a major role in how families address asthma.

For around-the-clock management of asthma symptoms and medications, download the Asthma Buddy App through the Apple App Store or Google Play, or text “ASTHMA” to 77-444.

Interested in sharing your ideas and perspectives on improving care at Children’s Health? Join the Youth and Young Adult Board (YAB).

Through the Family Advisor Network (FAN), Children’s Health partners with current and former patients ages 13-21.

Members of the board will provide their insight, experience, and guidance on the care of teens and young adults across Children’s Health. Meetings are held on a monthly to quarterly basis and you can attend both in person and virtually.
